CSR Requests
- Download a CSR request form
- Complete all required fields on the form.
- Fax to (781) 240-0381, or email to
csr-requests@dscicorp.com
LSR Requests
To have an order issued for a port out of a DSCI
on-net customer, follow these steps.
- Complete the CSR request form and fax or email as stated above.
- For simple porting (porting 1 number with a due date of 1 business day or greater), complete the Simple Porting Short Form. (Excel or PDF)
- For porting of multiple numbers, complete the standard LSR request form.(Excel or PDF)
- Email the form to lsr@dscicorp.com or fax to 781-240-0381.
