CSR Requests

  1. Download a CSR request form
  2. Complete all required fields on the form.
  3. Fax to (603) 286-4080, or email to
    csr-requests@dscicorp.com
LSR Requests
To have an order issued for a port out of a DSCI
on-net customer, follow these steps.
  1. Complete the CSR request form and fax or email as stated above.
  2. Complete the LSR request form.(Excel or PDF)
  3. Email the form to lsr@dscicorp.com or fax to 603-286-4080.
